Thank you for your interest in our free home visiting program for families, from pregnancy until the child goes to school. The program supports pregnant moms, their partners, other caregivers and their families.
What is the Healthy Babies Healthy Children (HBHC) Program?
A province-wide program that offers support to pregnant women & families with children from 0 - transition to school. Free, confidential and voluntary.
Provides support and information on topics such as pregnancy, breastfeeding, healthy eating, baby care, growth and development, parenting, as well as many other related areas.

By providing your information on this form, you consent to a member of our Healthy Growth & Development team to contact you. During this call, our Public Health Nurse will determine your eligibility to the Healthy Babies Healthy Children Program.
Personal information on this form is collected under the authority of section 5 of the Health Protection and Promotion Act, R.S.O. 1990, c.H.7. All information collected will be stored in a confidential and secure electronic database. This information will be used in the context of the Healthy Growth and Development program, Grand Erie Public Health in planning and delivering services for clients and their families.
